Pulmonary Function Testing
Objective assessment of lung function is critical for confirming the diagnosis and monitoring progression.
Spirometry and Reversibility
For older children, spirometry is the gold standard to detect reversible bronchial obstruction. A diagnosis of asthma is supported by a positive bronchodilator response, defined as an improvement in FEV1 of:
&\text{FEV}_1 \geq 12\% \text{ from baseline} \\
&\text{AND} \\
&\text{FEV}_1 \geq 200\text{ mL in absolute value}
\end{aligned}$$
. Other indicators of obstruction include an $\text{FEV}_1/\text{FVC}$ ratio $<80\%$ or a $\text{FEF}_{25-75\%} <65\%$.
** or **oscillometry**. Additionally, **bronchial hyperresponsiveness** can be assessed via methacholine challenge or exercise challenges, where a positive result is defined by an $\text{FEV}_1$ fall $\geq 10\%$ from baseline.
## Classification and Control Assessment
Asthma is classified by severity into **episodic** (occasional or frequent) or **persistent** (moderate or severe) based on the frequency of symptoms, nocturnal awakenings, and lung function variability.
The primary goal of management is **asthma control**. This is assessed by monitoring the following in the previous four weeks:
1. Daytime symptoms occurring $>2$ times per week.
2. Nighttime waking due to asthma.
3. Use of reliever medication $>2$ times per week.
4. Activity limitation due to asthma.
> **Exam Tip:** If a patient meets 1–2 of these criteria, they are considered "partly controlled." If they meet none, they are "well-controlled." If they meet 3 or 4, they are "uncontrolled".
## Pharmacological Management
Treatment aims to minimize symptoms, prevent exacerbations, and maintain normal lung function and activity levels.
### Inhaled Controller Classes
Maintenance therapy is tiered based on severity:
* **Inhaled Corticosteroids (ICS):** The first-line anti-inflammatory treatment (e.g., budesonide, fluticasone).
* **ICS-LABA:** A combination of ICS and a Long-Acting Beta-Agonist for patients uncontrolled on ICS alone.
* **ICS-formoterol MART:** A single combination used for both maintenance and relief (Maintenance and Reliever Therapy).
* **LAMA Add-on:** Long-acting muscarinic antagonists (e.g., tiotropium) may be added in selected uncontrolled cases.
### Biologic Therapies for Severe Asthma
For patients with severe, uncontrolled asthma despite optimized treatment, biologics may be indicated:
* **Omalizumab:** Anti-IgE, for severe allergic asthma.
* **Mepolizumab:** Anti-IL-5, for severe eosinophilic asthma.
* **Dupilumab:** Anti-IL-4R$\alpha$, for T2/eosinophilic inflammation.
### Delivery Devices
Selection of the device is age-dependent:
* **$<4$ years:** Metered Dose Inhaler (MDI) + spacer + mask.
* **4–6 years:** MDI + spacer + mask.
* **$>6$ years:** Dry Powder Inhaler (DPI) or MDI + spacer + mouthpiece.
## Differential Diagnosis: "Not Everything is Asthma"
Clinicians must remain vigilant when symptoms do not respond to standard therapy or present atypically.
### When to Suspect Other Conditions
Suspect non-asthmatic causes if symptoms began at birth, recovery is incomplete, or there is a poor response to treatment.
| Condition | Key Clinical Features |
| --- | --- |
| **Laryngotracheomalacia** | Inspiratory stridor, often presenting in infancy. |
| **Vocal Cord Dysfunction** | Inspiratory stridor caused by inappropriate vocal cord adduction; often confused with asthma. |
| **Congenital Malformations** | CPAM, bronchogenic cysts, or pulmonary sequestration; may present with recurrent pneumonia. |
| **Protracted Bacterial Bronchitis** | Chronic wet cough lasting $>4$ weeks, responsive to antibiotics. |
| **Cardiac Asthma** | Wheezing due to pulmonary edema from heart failure (e.g., VSD, PDA). |

> **Clinical Pearl:** If a patient is diagnosed with asthma but shows an "inspiratory" flattening on the flow-volume loop during spirometry, consider **Vocal Cord Dysfunction** rather than typical bronchial asthma.
