The integumentary system serves as the body's primary barrier against environmental insults, but it is also a frequent site of inflammatory activity. Inflammatory skin disorders can range from mild, self-limiting rashes to chronic, systemic autoimmune conditions. Understanding these disorders requires a keen eye for specific lesion patterns, such as plaques, vesicles, or "target" lesions, and an understanding of the underlying physiological triggers, including immune hypersensitivity and chemical irritation.

Pruritus and Clinical Manifestations

One of the most common symptoms across nearly all dermatological disorders is pruritus, or itching. The sensation of itching is transmitted from the skin to the Central Nervous System (CNS) via sensory nerve fibers and is often triggered by chemical mediators such as histamine.

While scratching may provide temporary relief, it often exacerbates the condition. Chronic scratching can lead to several secondary complications:

  • Excoriations: Superficial abrasions or scratches caused by rubbing.
  • Lichenification: Thickened, leathery skin with prominent skin lines resulting from chronic rubbing.
  • Secondary Infection: Breaks in the skin barrier can allow bacteria or fungi to enter.

To accurately describe these disorders, clinicians must identify specific lesion types. Common manifestations include macules (flat, <1 cm), papules (small, raised, <1 cm), plaques (raised, flat-topped, >1 cm), and vesicles (small, fluid-filled blisters).

Exam Tip: When distinguishing between a vesicle and a bulla, remember the size threshold. A vesicle is small, while a bulla is a fluid-filled blister larger than 1 cm.

Dermatitis: Types and Triggers

Dermatitis (eczema) is a broad category of inflammatory skin disorders characterized by pruritus, inflammation, and epidermal changes, often presenting with poorly defined borders.
